Request an Appointment
Our team members are available during normal office hours to help answer your questions and guide you through the process of scheduling an appointment.
Prior to any appointment, we do require a copy of your pet's medical history from any recent veterinary visits (when available) and will require you to complete an online form to allow us to create a medical record.
If you feel that your pet requires in-home euthanasia but your pet has not recently seen a veterinarian, we recommend that you start there. Alternatively, we may require that you set up a call with one of our team members to discuss the changes that you are seeing at home, and to determine if there are strategies that might be implemented to help your pet achieve an improved quality of life and spend more time with you. After all, we want to ensure that you get to spend as much quality time with your pet as possible!
Many times, we have received calls from families who have noticed changes in their pets that they believe indicate it is time to say goodbye. However, sometimes these changes are due to a treatable condition, or simply require some additional care to support them.
For those who wish to start this process online, we have included some information and the links to the necessary forms below. Simply click on the photo or title of the area that you are interested in to take you to the necessary online forms.
Once we have received your submission, a team member will reach out to you within one business day to arrange an appointment. If you require more immediate assistance, please call us at 289-987-7297.
